Eligibility

  • The plan is good only at Northside Family Dentistry. Discounts do not apply for services requiring referral to the specialist.
  • Upon enrollment, the plan is effective immediately and is good for 12 months from signup date.
  • Eligible dependents include spouse and children residing with you through age 25.
  • All patients are subject to Northside Family Dentistry office policies.
  • In-Office Dental Plan cannot be combined with dental insurance or any other offers.

Exclusions

  • If you choose to extend your payment for treatment with CareCredit, the treatment discount is reduced to 5% due to merchant fees.
  • Periodontal therapy including scaling and root planning is covered by the 10% discount.
  • Treatment initiated prior to enrollment is not eligible for discount.
  • Treatment fees are guaranteed for 3 months from date quoted by the office.
  • Plans and fees are subject to change yearly.
  • Northside Family Dentistry reserves the right to discontinue plan for any member at any time.
  • No-shows or cancellations without 24-hours notice can lead to the patient being dropped from this plan without a refund.

What if I have a dental emergency?

Our goal is to be available for you when you have a dental emergency. Dental emergencies can include facial trauma that damages the structure of the teeth or jaw, loss of a permanent tooth, severe tooth pain, or signs of infection. If you believe you have a dental emergency, please give us a call right away at (931) 787-1718.

Do you see patients of all ages?

Our patients range in age from 3 to 103! We gladly see patients of all ages at Northside Family Dentistry in Crossville.

What forms of payment are accepted?

We accept cash, check, Visa, MasterCard, Discover, and HSA Cards. We also accept most major insurances. If any of these options aren’t for you, we provide an In-Office Savings plan as well.

How does your practice accommodate people with disabilities?

Our practice offers handicap accessible amenities including convenient ramps from the parking lot, wide walkways, automatic doors, entrance ways that measure 32” or greater, and a designated wheelchair accessible operatory. We also have staff on hand available to read aloud documents for the illiterate or blind, and have braille in the waiting area and in all restrooms. Our website is also ADA compliant. What to Expect For Your Next Dental Visit

Here’s what you need to know for your visit:

We’ve added enhanced infection control procedures.
We’re asking patients to wait in their car and we’ll contact you when it is time to come in.
We’re taking the temperature of our patients and team to ensure temperature is below 100.4°F.
We’ll reach out to you before and ask a few screening questions. We’ll ask you again when you come to see us.
Please limit extra companions during your visit. Only bring people who have an appointment so we can reduce the number of people in the office.
We look forward to seeing you again and are happy to answer any questions you may have about the steps we take to keep you, and every patient, safe at our practice.
